SUBJECT: Reorganization Plan Checklist and Cover Page

In order to assist Reorganization Planning Committees (RPCs) and their member School Administrative Units (SAUs) in determining the completeness of their Reorganization Plans, the Department has developed a two-page checklist that includes fields for barriers and assistance needs. These forms will assist the RPCs and SAUs in determining the status of the plan elements and allow the Department to evaluate the Reorganization Plan’s status in the event that an incomplete plan is submitted.

We have also developed an updated cover page for Reorganization Plans. This form will allow for one plan to be submitted by all school administrative units involved in the RPC, eliminating the possibility of a non-reciprocal plan, and should also contribute to a more streamlined approval process.

The Reorganization Planning Committee should complete the updated cover page and attach it to the front of the Reorganization Plan. Please be sure to include the requested contact information. The checklist should also be attached to the front of the plan as a guide to which parts of the plan are complete, in progress, have not been started, are a noted barrier, and/or are areas that the RPC needs assistance in completing.

These forms are available online in three formats: .rtf .pdf, and .doc version (the later version allows the form to be completed electronically and printed).
