Please include your full name, date of birth or Maine driver's license number, and current mailing address. If you are requesting a record, the fee is $5.00. If you wish the information to be faxed to you, please include the fax number and an additional fee of $2.00. There is an additional fee of $1.00 for a certified copy of the record.
