RECORDS MANAGEMENT FORMS
for use by Executive and Legislative Agencies*

APPLICATION FOR RECORDS RETENTION SCHEDULE

You will need scroll bars to navigate these forms in Microsoft Word. Please check your Word settings to make sure you have these showing. If not, or if you're unsure what these are, open the TOOLS menu in Word - click OPTIONS - select the VIEW tab - and then make sure both Horizontal Scroll Bar and Vertical Scroll Bar are checked. Please feel free to call us at Records Management if you need help with this, because it makes all the difference in being able to complete the forms easily.

This form should be typed (or completed on your screen after you download it), printed, and signed by your Departmental Records Officer or Agency Head. It will be retained permanently at the Maine State Archives, so we do need the hard copy with signature. Please mail completed form to 84 State House Station.

RECORDS SERIES INVENTORY

This form should be typed (or completed on your screen after you download it), printed, and signed by your Departmental Records Officer or Agency Head. It will be retained permanently at the Maine State Archives, so we do need the hard copy with signature. Please complete a Records Series Inventory Form for each series you are submitting, and please include samples of the records to be scheduled (photocopies are fine, and you may use a marker to black out personally identifiable information if the records are confidential). Several Record Series may be listed on the same Application for Records Retention Schedule. Please mail completed form(s) with samples to 84 State House Station.

TRANSMITTAL OF RECORDS and TRANSMITTAL OF RECORDS CONTINUATION SHEET

These forms may be typed, completed on your screen after downloading, or LEGIBLY hand printed. They require a Records Officer's signature unless it is being submitted by e-mail from a State address. If e-mailing, please submit to : Michael.Lawrence@maine.gov

PACKING LIST

Thi s form should be comp leted (as many pages as necessary) and enclosed in any box packed for transfer to the State Records Center or the Division of Archives Services. You should keep a copy for your own records, listing each file in the box. If you have questions about this, please contact Michael.Lawrence@maine.gov .

RECORDS MANAGEMENT EXPENDITURE REQUEST

This form may be typed, completed on your screen after downloading, or LEGIBLY hand printed. It requires a Records Officer's signature unless it is being submitted by e-mail from a State address. If e-mailing, please submit to : Barry.Marshall@maine.gov

REQUEST FOR REFERENCE SERVICE

Please download, complete and then e-mail to Michael.Lawrence@maine.gov , or print out and fax to 287-5739.

APPLICATION FOR RECORDS CENTER USE CARDS

Please download, complete and then e-mail to Michael.Lawrence@maine.gov (but only if you are the Records Officer at your agency). You may also, of course, print it and fax to 287-5739 or mail to 84 State House Station. Requests not received from or signed by an authorized Records Officer will not be processed.
