Athletic Trainer
"Athletic trainer" means a person licensed by the department to use that title after meeting the requirements. (Please refer to 32 MRS Chapter 127-A §14356 for Exemptions.)
How to apply

- State Police Criminal Background Check ("SBI") fee of $21.

- Check in the amount of $221. payable to "Maine State Treasurer" or credit card authorization
- Completed application
- TRANSCRIPTS - Provide a transcript(s) that contain college or university names and curriculum in athletic training, or provide proof of completion of an athletic training education program approved by the National Athletic Trainers Association
- CERTIFICATE - Provide a copy of certificate if currently certified by the National Athletic Trainers Association or proof of having passed the National Athletic Trainers Association Board of Certification Examination.

- Completed Application
- Your National Athletic Trainers Association Certification of Continuing Education (NATA) number, as it appears on your current card.
- Completion of criminal conviction question by indicating "Yes" or "No," and submission of documentation that lists date(s) and crime(s) with a copy of the court judgment(s). If your answer is "Yes," then you must mail the completed renewal application and the necessary fees, along with a copy of the court judgment. You must also submit a letter listing the date of the conviction, the crime committed, and explaining the circumstances surrounding the conviction.
- A late fee of $50 is assessed for licenses renewed after the license expiration date
- A person who submits an application for renewal more than 90 days after the license expiration date is subject to all requirements governing new applicants and is required to reapply with the original license application, documentation and fees.

The Department must verify that you have met all the conditions for renewal before your license is renewed. Online submission of your renewal application should not be construed as automatic renewal of your license.
