If applying, you must submit:

• A completed Application for Licensure;
• Official transcript of a sufficiently related earned bachelor's degree;
• Three current (within the past year) letters of professional recommendation, two of which must be from licensed social workers(at any level of licensure from any state);
• Evidence of employment in a social work setting;
• NPDB/HIPDB Self-query report(s); and
• Agreement to provide Consultation form.

• Biennial renewal fee $70

• Can only renew once

• A late fee of $50 is assessed for licenses renewed after the license expiration date.
• A person who submits an application for renewal more than 90 days after the license expiration date is subject to all requirements governing new applicants and is required to reapply with an original license application, documentation and fees.
• Renewal applications are mailed approximately 60 days prior to the license expiration date.
• Continuing Education: 25 hours of continuing education pursuant to board rules.
