Summary: Licensee, an Oregon resident, was working at a Maine hospital on a short-term basis to provide temporary radiologic services due to periodic shortages of staff in the radiology department. One afternoon licensee failed to appear for a scheduled work shift without notice to anyone at the hospital. Licensee's supervisor then went to the licensee's apartment which was owned and furnished by the hospital in order to investigate the situation She discovered that licensee had vacated the premises and apparently had removed some of the linens and furnishings, including the television and VCR. Licensee never contacted the hospital thereafter to explain his actions.

Licensee's unannounced absence from work had an adverse impact on both the patients and small staff at the hospital. For example, some patients were rescheduled for x-rays and other tests were postponed. The radiology staff, already shorthanded, suffered morale problems since several were forced to work overtime for a period of six weeks until a replacement for the licensee was hired. The board reprimanded licensee, fined licensee $3000, and ordered licensee to pay $300 hearing costs. The board noted that if licensee had possessed an active license at the time of hearing, that license would have been revoked by the board. The board strongly discouraged licensee from reapplying for licensure.

Licensee is on probation for up to one year, which probation may terminate upon licensee's successful completion of one of the following options within one year of the initial date of probation: (a) provide documentation of a passing score on an entry level examination in a discipline in which licensee has not previously been licensed; (b) provide documentation of a passing score on an advanced level ARRT examination; or (c) obtain 12 continuing education units AND the balance of the 24 continuing education units which were due for the previous license biennium. If licensee fails to complete one of the foregoing options within the one year probationary period, licensee shall immediately and voluntarily surrender the license to the board. Upon licensee's timely fulfillment of one of the foregoing options, the probation shall terminate.

The condition of probation is that licensee will successfully complete one of the following options during probation: (a) provide the board with documentation of a passing score on an entry level examination in a discipline in which licensee has not previously been licensed; (b) provide the board with documentation of a passing score on an advanced level ARRT examination; or (c) obtain and provide documentation of 12 continuing education units and the balance of the 24 continuing education units which were due for the previous license biennium. Upon timely completion of one of these three options, probation shall terminate. If licensee fails to provide the board with satisfactory evidence of completion of any one of the three options, licensee shall immediately and voluntarily surrender her license.

The board agreed to renew the license. Licensee agreed that if she fails to provide the board with satisfactory evidence of completion of any one of these three options within the 1-year probation, she will immediately and voluntarily surrender her license.
