• Criminal History Record Check $15.00 until June 30, 2008.
• Effective July 1, 2008, the Maine State Bureau of Identification is adopting a rate changes. The fee for criminal background checks will increase to $21.00. Applications received on or after July 1, 2008 are subject to the $21.00 fee for the criminal history records check.
  

• Annual Cycle - annually at the end of the month based on the first licensure date.

The following is the application procedure:

• Complete the application for registration. Include appropriate fee (check or money order payable to Treasurer State of Maine, VISA, or MasterCard)
• Submit both application and form, along with the appropriate fees together to the following:

• Annual renewal fee is $25.00.

• This registration renews annually at the end of the month based on the first licensure date.

• A late fee of $50.00 is assessed for licenses renewed after the license expiration date.
• Licenses may be renewed up to 90 days after the date of expiration upon payment of a late fee of $50 in addition to the renewal fee. Any person who submits an application for renewal more than 90 days after license expiration date is subject to all requirements governing new applicants and is required to reapply with original license application, documentation and fees.
• Renewal reminders applications are mailed 60 days prior to the license expiration date. It is the licensee's responsibility to keep the Board informed of the current license address and/or to see that the license is renewed absent the renewal application.
