Occupational Therapist

"Occupational Therapist" means an individual who has passed the certification examination of the National Board for Certification in Occupational Therapy for an Occupational Therapist or who otherwise meets the requirements and who is licensed to practice Occupational Therapy in the State.

  • Application Fee of $60
  • Licensure Fee of $80
  • Criminal History Records Check Fee of $21

Term
  • Biennial (Fixed renewal date of March 31st of odd years)
Requirements
  • Application with all sections completed;
  • Verification of certification form completed and signed by NBCOT;
  • Verifications of licensure from sending state(s) (if applicable);
  • Two appropriate reference forms addressing ethical practice; and
  • NPDB/HIPDB Self-query report(s).

Renewal fees and information

Fees
  • Annual renewal fee $80
Requirements
  • A late fee of $50 is assessed for licenses renewed after the March 31 license expiration date.
  • Renewal reminders are mailed approximately 60 days prior to the license expiration date.
  • A person who submits an application for renewal more than 90 days after the license expiration date is subject to all requirements governing new applicants and is required to reapply with an original license application, documentation and fees.
  • Continuing Education: 36 hours of continuing education pursuant to board rules.

  • Fill out the online renewal application and submit.

    The Department must verify that you have met all the conditions for renewal before your license is renewed. Online submission of your renewal application should not be construed as automatic renewal of your license.
