Temporary Occupational Therapy Assistant

A temporary license may be granted to a person who has completed the education and experience requirements. This temporary license allows the person to practice occupational therapy under the supervision of a licensed occupational therapist. This license is valid until the results of the next scheduled national examination taken by the person are available to the Board. Please see Title 32, Chapter 32, § 2278 for complete information.

How to apply

Fees
  • Application Fee of $60
  • Licensure Fee of $20
  • Criminal History Records Check Fee of $21

Term
  • A temporary license is valid for six (6) months or until the results of the examination are available.
Requirements
  • Applicants for temporary licensure must submit:
    • Application with all sections completed;
    • Completed Supervisor's Affidavit;**
    • Official transcript or completed verification of education form;
    • Two appropriate references forms addressing ethical practice;
    • All applicants must submit documentation of NBCOT approval to sit for the certification examination; and
    • NPDB/HIPDB Self-query Report(s).
    **The Board must be notified of any change in the temporary licensee's supervisor. Such notification shall be in the form of a signed supervisor affidavit from the new supervisor and mailed directly to this office at the above address.

Renewal fees and information

(Note: Renewal is not automatic. Please refer to Board Rules, Chapter 3, Section 2(2) for more information.)

Requirements
  • The temporary license of a person who has failed the examination may be renewed one time at the discretion of the board. Please refer to Board Rules, Chapter 3, Section 2(2) for more information.
