Occupational Therapy Assistant

"Certified Occupational Therapy Assistant" means an individual who has passed the certification examination of the National Board for Certification in Occupational Therapy for an occupational therapy assistant or who was certified as an Occupational Therapy Assistant prior to June 1977 and who is licensed to practice Occupational Therapy under the supervision of an Occupational Therapist.

Term
  • Biennial (Fixed renewal date of March 31st odd year)
Requirements
  • Application with all sections completed;
  • Verification of certification form completed and signed by NBCOT;
  • Two appropriate reference forms addressing ethical practice;
  • Supervisor's Affidavit completed and signed by Maine Licensed OTR; and
  • NPDB/HIPDB Self-query report(s).

Renewal fees and information

Fees
  • $70

Requirements
  • Completed Application.
  • A late fee of $50 is assessed for licenses renewed after the March 31 license expiration date.
  • A person who submits an application for renewal more than 90 days after the license expiration date is subject to all requirements governing new applicants and is required to reapply with an original license application, documentation and fees.
  • Renewal applications are mailed approximately 60 days prior to the license expiration date.
  • Continuing Education - 36 hours of continuing education pursuant to board rules.
