Residential Care Facility Administrator
"Residential Care Facility Administrator" is an individual who is charged with, and has responsibility for, the general administration of a Residential Care Facility. A "Residential Care Facility" refers, for this purpose, to a Level IV Residential Care Facility having a licensed capacity for seven or more residents, and whose services provided fall within the definition of a medical care facility other than a hospital.

| Requirements |
- Completed and signed application for licensure;
- Written documentation that the applicant meets the requirements for a Residential Care Facility Administrator as outlined in Chapter 3, Section 1(B) of the Board Rules; and
- Two (2) written character reference letters indicating that the applicant is of good record and reputation for honest and reliable conduct in personal and business affairs; and
- NPDB/HIPDB Self-query report(s).
Upon submission and approval of the above requirements, you will be issued a license as a Residential Care Facility Administrator.
If you are applying for licensure by endorsement, you must meet the requirements of Chapter 6 of the Board Rules. |

Renewal fees and information

| Requirements |
- A late fee of $50 is assessed for licenses renewed after the June 30 license expiration date.
- Renewal reminders are mailed approximately 60 days prior to the license expiration date.
- A person who submits an application for renewal more than 90 days after the license expiration date is subject to all requirements governing new applicants and is required to reapply with an original license application, documentation and fees.
- Continuing Education - 12 hours of continuing education pursuant to board rules.

The Department must verify that you have met all the conditions for renewal before your license is renewed. Online submission of your renewal application should not be construed as automatic renewal of your license.
