Massage Therapist
"Massage Therapist" means a person who provides or offers to provide Massage Therapy for a fee, monetary or otherwise.
"Massage Therapy" means a scientific or skillful manipulation of soft tissue for therapeutic or remedial purposes, specifically for improving muscle tone and circulation and promoting health and physical well-being. The term includes, but is not limited to, manual and mechanical procedures for the purpose of treating soft tissue only, the use of supplementary aids such as rubbing alcohol, liniments, oils, antiseptics, powders, herbal preparations, creams or lotions, procedures such as oil rubs, salt glows, and hot or cold packs or other similar procedures or preparations commonly used in this practice. This term specifically excludes manipulation of the spine or articulations.

- Provide a copy of high school diploma or its equivalent
- Provide proof of completion of current Adult CPR and Basic First Aid
- EITHER --
- Provide evidence of having passed either the examination sponsored by the: (1) National Certification Board for Therapeutic Massage and Bodywork or (2) Federation of State Massage Therapy Boards (MBLEx).
- OR --
- Provide a transcript that outlines a course of training consisting of 500 hours meeting the required course content.
Note: If you did not graduate from a school with an approved program, then please also submit a "Core Curriculum Requirements" form.

- Provide a Healthcare Integrity and Protection Data Bank Report -- Not the Self-Query. You send the Self-Query to to the National Practitioner Data Bank / Healthcare Integrity Protection Databank to obtain the report; then, you include the report with the application you send to us. Please do not submit your Massage Therapy license application to this office until you have received the NPDB/HIPDB report to include with it.

Renewal fees and information

- Check or Credit Card Authorization for payment of renewal fee;
- Completed Application;
- A late fee of $50. is assessed for licenses renewed after the license expiration date; and
- A person who submits an application for renewal more than 90 days after the license expiration date is subject to all requirements governing new applicants and is required to reapply with original license application, documentation and fees.
