Dietitian
"Licensed Dietitian" means a person who practices Dietetics and who is licensed as a Dietitian. "Dietetics" means the professional discipline of assessing the nutritional needs of an individual, including recognition of the effects of the individual's physical condition and economic circumstances, and the applying of scientific principles of nutrition to prescribing the means to ensure the individual's proper nourishment and care.

- Completed and signed application;
- Official transcript;
- Proof of completion of Clinical Experience
- Official notice of examination passage or current copy of Commission on Dietetic Registration (CDR) wallet card;
- Reference letter, which addresses professional ethical standards written by a professional with knowledge of nutrition practice. The recommendation should not be an employee under the applicant's direct supervision;
- NPDB/HIPDB Self-query report(s);
- Verification of licensure from state(s) in which you hold or previously held licensure or registration; and
- Statute and Rules from sending state.

Renewal fees and information

| Requirements |
- A late fee of $50 is assessed for licenses renewed after the December 31st license expiration date;
- A person who submits an application for renewal more than 90 days after the license expiration date is subject to all requirements governing new applicants and is required to reapply with an original license application, documentation and fees; and
- Continuing Education - 15 hours of continuing education pursuant to board rules.
