Establishment Shop

Establishment or shop means a beauty shop or salon, cosmetology shop or salon, barber shop or salon or hair styling shop or salon, or any premises, structure, building or part of a building where any activity licensed under this chapter is practiced.

How to apply

  • Initial license fee $20.00
  • Annually October 31st.

Complete and submit the Application for Establishment with the following information:

  • Appropriate fee made payable to Treasurer, State of Maine or if paying by credit card submit the authorization form with your application;
  • Directions to your establishment. Make directions as clear as possible noting any identifying signs landmarks or structures that will assist in locating your establishments.
  • Floor plan of your establishment. Mark entrances, exits, purpose of each room or area, and location of the required public rest room. Reminder: all establishments must have a utility sink or shampoo bowl in addition to the sink that is in the restroom.
  • Electrical Inspection. You must have your shop inpected by a master license electrician or the town/city code enforcement officer. Establishments that apply for a change of ownership are not required to submit documentation of compliance if the existing establishment does not have any electrical work or renovations done and the initial inspection was done within 5 years from the change of ownership. Questions about electrical installations should be directed to your electrical contractor.
  • Plumbing nspection. You must have your shop inpected by a master license plumber or the town/city code enforcement officer. Questions regarding plumbing installations should be directed to the local plumbing inspector in your city or town.
  • If you have a private water supply, it must be tested for bacteria and coliform prior to the licensure of your establishment. Private sewage systems shall meet the standards set by the Department of Human Services.

Please pay particular attention to Chapter 3 of the Program rules. Chapter3 will provide you with substantial information in setting up your business, sanitary and safety compliance requirements. Inspections conducted by the inspectors track the provisions of Chapter 3.

Upon receipt, your information will be evaluated and a license will be issued and mailed to you if approved. The self inspection check list program is not intended to circumvent compliance with Program laws and rules. This process is intended to ensure shops meet the minimum requirements to open for business more quickly. A physical inspection of your shop will be done in approximately 90 after your license is issued.

This agency can not be held responsible for advanced bookings of clientele and/or advertisements placed. Your establishment can not open for business until your license has been issued.

Renewal fees and information

  • Annual renewal fee is $20.00.
  • This license renews annually October 31st.
  • A person who submits an application for renewal after the expiration date is subject to a late fee of $50.00;
  • More than 90 days after license expiration date is subject to submitting a new application with appropriate fee and a late fee of $50.00 plus penalty fees of $25.00.
  • Reinstatement Application

Renewal reminders are mailed approximately 60 days prior to the license expiration date. It is the licensee's responsibility to keep the Program informed of the current license address and/or to see that the license is renewed timely even if you do not recieve a renewal reminder.

    The Department must verify that you have met all the conditions for renewal before your license is renewed. Online submission of your renewal application should not be construed as automatic renewal of your license.
