Summary: On one set of plans, licensee failed to properly supervise employees, one of whom committed forgery by signing the name of an engineer on each page of a set of plans which the engineer never saw or approved. Licensee knew or should have known of this forgery when he signed the architectural plans in his own name and submitted the plans with the forged signatures to building officials and others. On a second set of plans, licensee listed an engineer as a member of the design team, but the engineer did not sign the plans. Licensee signed the plans and certified that they were prepared by him, or under his supervision, in spite of not being licensed to practice engineering. Licensee was disciplined in Minnesota for these incidents. Licensee affixed his seal and signature to a third set of plans, which plans were not prepared by him, or in his office, or under his direct supervision. Licensee was disciplined in Nevada for this incident. For the four rule violations arising from these three incidents licensee was issued four concurrent suspensions of his license to practice architecture until 6/30/00, four civil penalties totaling $1000, and was put on probation for four years upon the conclusion of the suspensions. During the period of probation, licensee may not practice architecture in Maine without being supervised by a Maine-licensed architect residing in Maine.

Summary: Licensee attached his architectural seal to and was compensated for plans for a church in Arkansas that were neither prepared by him nor under his direct supervision. Licensee was issued a reprimand and a $2940 civil penalty by the Arkansas State Board of Architects. Licensee was also issued a reprimand by the Maine board in addition to the Arkansas penalty.

Summary: Licensee presented proposal to prospective client that (1) did not accurately represent the work for which the licensee's firm was claiming credit, (2) did not accurately represent the responsibility claimed to have been exercised by the firm's agents with respect to the listed work, and (3) created an inaccurate but reasonable perception that the Director of the Maine Historic Preservation Commission was part of the licensee's project team, or one of its paid consultants. Licensee was issued a reprimand. Licensee was also ordered to submit to the board copies of responses submitted by licensee, or any entity in which licensee holds an equity interest, to requests for qualifications or requests for proposals for architectural design services for public entities for any architectural services, or to private entities for architectural services in connection with non-residential construction, pursuant to conditions set forth in the consent agreement.
