Board of Alcohol & Drug Counselors - Applications & Forms

 

Applications for Examinations

• Application for Eligibility to Qualify for the ADC Examination
• Application for Eligibility to Qualify for the Advanced ADC Examination
• Application for Eligibility to Qualify for CS Examination

The above applications are solely for determining a persons eligibility to qualify for for an exam. It does not qualify an applicant for licensure at this time.

Once an exam candidate has passed an exam, they would be required to submit a separate application for licensure.

Applications for Licensure

• Application for Alcohol & Drug Counseling Aide
• Application for Licensure for Certified Alcohol & Drug Counselor (CADC)- once you have taken and passed the IC&RC ADC and Advanced ADC examinations
• Application for Licensure for Licensed Alcohol & Drug Counselor (LADC)- once you have taken and passed the IC&RC ADC and Advanced ADC examinations
• Application for Licensure for Certified Clinical Supervisor (CCS) - once you have taken and passed the IC&RC CS examination
• Application by Endorsement for CADC, LADC, CCS License
• Application for Reinstatement of an Alcohol & Drug Counseling Aide - 91 days to 2 years
• Application for Reinstatement of CADC, LADC, CCS License - 91 days to 2 years
• Application for Reinstatement of CADC, LADC, CCS License - more than 2 years

General Forms

• Verification of Licensure Request Form
• Replacement License Request Form
• Continuing Education Program Approval Form
• Credit Card Authorization Form
  To authorize payment of fees by Visa or MasterCard.
