Online Data Reports

The reports listed in the table below must be completed using the Bureau of Insurance's online reporting system. In order to enter information into the system, you need your company's Maine license number and access code. If you do not have your access code, please contact the person listed below for your report. By accessing these reporting forms, you acknowledge that you have the proper authorization to file these reports on behalf of your company.

After completing and submitting your company's data, you will receive a confirmation email with a pdf file containing your reported information. If you discover that your data needs to be edited after it has been submitted, contact the appropriate person listed below.
