The insured requested a hearing regarding the nonrenewal of his homeowners policy for failure to provide documentation relating to construction of an addition.

Held: For the insured. Hingham testified that its agent was asked to secure from the insured a copy of the building permit and inspection certificates from code enforcement authorities upon completion of the construction. The company failed to document that the agent relayed the request to the insured, or establish how the alleged failure to provide them related to the insurability of the property. In addition, the reason for nonrenewal specified the failure to provide certificates of insurance from contractors, but there was no indication those documents were requested.
