Active and non-active licenses
must be renewed annually on or before April 1st of
each year.
License renewal applications
will be mailed approximately sixty (60) days prior
to April 1st. It is the licensee's responsibility
to keep the Board informed of the current address.
A late fee of $75 is assessed
for renewal applications received after the expiration
date.
A person who submits an application
for renewal more than sixty (60) days after the license
expiration date is subject to all requirements governing
new applicants.
Documentation of 25 hours of
continuing education is due at the time of renewal.
At least fifteen (15) hours of continuing education
must be in diagnosis and treatment of ocular diseases.
A maximum of two (2) hours in practice management
and five (5) hours on-line courses.
The Board may waive the
continuing education requirement in cases of illness
or undue hardship.
