If you require verification of your Maine license to practice as an osteopathic physician or of your Maine license to practice as a physician assistant working under the primary supervision of an osteopathic physician, please forward your written request (or the form provided by the state to which you are applying) to this address*:

There is a fee of $15 for each request. Please make your check or money order payable to Board of Osteopathic Licensure. The Board does not accept credit card payments.
