Home > Licensure > Permanent Application Permanent ApplicationDear Applicant: Attached is an application for a permanent license to practice osteopathic medicine in the State of Maine. Please submit notarized photocopies of your medical school diploma, internship and residency certificates. A form is attached to be sent to any state in which you have ever been licensed; this includes states where you may have held only a training or temporary license. The Certificate of Medical Education must be mailed directly to the medical school which they will, in turn, mail directly to the Board office. The fee for an endorsement application is $350 (check or money order - do not send cash). If you took the National Boards, FLEX, or a state exam, please request that the agency forward, directly to this office, a transcript of your scores. Also, on the application please be sure that you provide complete names and full addresses of all hospitals or institutions where you have practiced either in a post-graduate program or in a private practice setting. If you answer "yes" to any question in the Personal Data section, you must provide the Board with full details. If you wish to utilize the Federation of State Medical Boards Credentials Verification Service (FCVS), you may download the packet from www.fsmb.org. If you are unable to do so, please notify the Board office and the materials will be mailed to you. You are not required to utilize the FCVS for licensure with this Board. The normal review process should take no longer than 90 days from receipt of application. You will hear from this office as soon as a decision regarding your application has been made. If you do not have Adobe Acrobat Reader you can download if for free at www.adobe.com or you can call the office at the number listed below and we will send an application to you.
