(#1752, Waterville, ME) - 1990 Consent Agreement regarding Dr. Amalfitano's practice of medicine. The Agreement was terminated in 1995. Amalfitano, Albert R., D.O. - 1998 Consent Agreement regarding improper prescribing procedures. This agreement was amended in September 1998 to include a letter of warning. Amalfitano, Albert R., D.O. September 1998 Amendment to Consent Agreement requiring 1) that the licensee take the SPEX examination upon completion of a remediation course; 2) 10 patient charts reviewed by a physician reviewer in the same practice specialty; 3) letter of warning (as noted above); 4) referral of patients with addictions to a detox program and/or referred to a new physician, pain clinic or emergency room. III drugs to patients, except those with 1) legitimate and documented pain from a dx of terminal cancer and, 2) those with a documented situation of acute pain for which the prescription can be for no more than a 30 day supply, no subsequent Sch. III prescriptions may be written. Licensee must assist those currently receiving prescriptions for Sch. III drugs to transfer prescribing to another physician or taper the patient off of the Sch.
