This form is for the first time you submit your school information. Please use the edit form for changes.

Please include information for only one school per form. If you supervise more than one school or split your position between schools, please add that in the comment box (supervising librarian with your name) BUT complete separate forms for each school. The directory will list ALL school libraries.

If you do NOT have a fax #, please write "none' in this required field.
