ATM E-rate Information

ATM (Schools, MSL and BPL)

  • Form 470 is done by DOE
  • Technology Plan needed
  • Multi-year contract is signed
  • Forms 471, 486, 472 (BEAR) are done by each school
  • Be aware of all form deadlines!
  • Have a copy of your contract
  • On Form 471, Item 21 Attachment –ATM is Digital Transmission Service
  • Wait for Funding Commitment Decision Letter (FCDL)
  • File Form 486 once FCDL is received (after July 1)
  • Invoice using Form 472 (BEAR)
  • Remember MSLN Reimbursement Form for additional funds reimbursement
