The Maine State Rehabilitation Tax Credit Reporting Form must be submitted in order for applicants to claim the state tax credits. The information provided on this form will be summarized in a report to the state legislature in 2013.

Part A of the form must be submitted with Part 2 of either the Federal Historic Preservation Certification Application, or Part 2 of the Small Project Rehabilitation Certification Application.

Part B of the form must be submitted with Part 3 of either the Federal Historic Preservation Certification Application, or Part 3 of the Small Project Rehabilitation Certification Application.
