State Vendor Code Form
This form is to be used by attorneys and other providers when applying for a vendor code or making modifications to existing vendor codes. There is also attached below there is an instruction sheet to assist with filling out the vendor form. Download the form from the State Controller website.
Direct Deposit Form
This is the form for attorneys and other vendors to fill out in order to have their payments from the State of Maine directly deposited into their accounts.
Judicial Branch's MEJIS Form
This form is for attorneys to use when needed to make changes to their contact information with the MEJIS computer system used by the Judicial Branch.
Witness Fee Request Form
This form is to be filled out by witnesses who have appeared in court under a subpoena issued by a MCILS rostered attorney on behalf of an indigent client and are requesting payment for the witness fee and mileage.
Preliminary Motion for Assignment of Counsel, Affidavit and Release
The top part of this form is a Motion/Affidavit to be filled out by the defendant or party who is requesting assigned counsel. The form should be filled out with the assistance of a financial screener in courts with financial screeners. The bottom of the form is an Order to be used by the Court when rendering a decision on the Motion. The other form is a Social Security Form which must be filled out and filed along with the Motion/Affidavit.
Child Protection Financial Affidavit
This is the financial affidavit form that should be filled out and filed by the party requesting assigned counsel in a child protective case. The form should be filled out with the assistance of a MCILS Financial Screener in courts that have financial screeners.
ILAP Consultation Process
The attachment below outlines the procedure that MCILS attorneys should follow when seeking ILAP's assistance with immigration issues related to the representation of their client(s) that are not born in the United States.
