2008 Annual Report Cover Letter

Hello Maine Wildlife Rehabilitator Permit Holder,

I hope this letter finds you well and having had a productive year.

As you are aware, reporting requirements to maintain your rehabilitation permit include submitting an annual report to Maine Department Inland Fisheries and Wildlife Augusta office.

To help improve efficiencies, an electronic version of this form is available on line and electronic filing is encouraged.  If filing electronically, please return the completed Wildlife Rehabilitation Annual Report to Karen.Estabrook@maine.gov. 

Failure to submit an annual report may result in the revocation of your permit, and would require the full application process, including supporting documentation and testing, to acquire a new permit.  Please be sure to send in your reports on time.

If you have any questions about your reporting requirements please contact me before January 31, 2009.
