Our Mission
The Maine Committee on Transition supports youth with disabilities in making meaningful choices as they transition to adulthood by promoting collaboration within communities.
What is good transition planning?
Define their dreams.
Identify strengths, needs, opportunities and worries.
Draft strategies, goals and outcomes.
Write and implement a plan based on the transition planning areas that will lead to their dreams.
What Schools Do!
Start Transition Planning by age 14.
Actively involve students.
Use outcome oriented process.
Use a team approach.
Include as team members the student, family, school personnel, representatives from community and state agencies/organizations.
Identify transition needs and incorporate them into the school program.
What We Do Through Our Regional Programs!
Information and referral.
Train school-based teams to use student-centered planning.
Help people make connections.
Coordinate access to programs and services.
Design talks and training just for you!
