Using the Preparation for Transitions Flow Chart

The flow chart is designed for the use of
teams planning a students high school program and beyond.
The suggested process for its use
includes:
Step 1 Discussion of
Post-secondary objectives in the five areas identified above the chart.
Step 2 Identification of the program areas to be
discussed by the team (first column on
both pages.
Step 3 A review and determination of needed
classes, programs and services to reach
the post-secondary goals. This review should begin at Year 4 (fifth column) and
proceed right to left year by year in order to ensure seamless connections
across time and readiness for success beyond the students school program.
