Fire Sprinkler Permit Application FormThe following application forms were created in AutoCAD 2004. If your version of AutoCad is prior to AutoCad 2004, then you will probably not be able to open the DWG version of the file. If you do not have AutoCAD, the application forms are also available as PDFs. If you do not have Adobe Reader, you will need to Get Adobe Reader Free to view the PDF files.
Downloading .DWG files using MSIE5.0 or greaterPlace your cursor on the appropriate link and double LEFT click and follow the instructions for saving the file. If that fails to work try the following : Place the cursor on the appropriate link and RIGHT click, you will see a dialog box, choose "SAVE AS" and assign the file a name and location of your choosing. After the file has downloaded you should be able to open the file at that location with your application. If you have AutoCAD, then you also have the option of filling out the form online before you print it, or to fill it out electronically, and then return it as an email attachment. This is the preferred way of the two methods, because then I can store it electronically. The check can be sent separately through the regular mail. The email address is on the forms. The best way however is to submit for a permit online, (go back out of this page, then click on "Online Permit Submittal"). When submitting through that link a permit application form is not even needed. The procedure will ask you basic questions about the job, giving you choices to click for your answers. That info is imported into my database as the job record, saving me data entry as well. You can attach electronic plans and calcs using the online permit submittal, and pay for the permit fee with a credit card too. All of the contractors who have tried the online permit submittal process prefer to use it because it saves them time and money, and is easy to use. Remember to throw away all of your old forms, and to print new first-quality forms from here whenever you need them. (Prior forms will not be accepted.) And don't forget that it is always good to have a backup file. To fill out the forms electronically, (only available with DWG files), click on the form you want above.(Be sure the form is in "Model Space").Next type the ATTEDIT command, which has the alias "ATE". Then click the "Enter" button, then click on any type that is on the form. You should get a dialog box pop up. If you don't, then try clicking on the form again until it does. Answer the questions in the dialog box by typing the answers in the space provided. Just answer the questions that apply. There is no need to type in any "No" answers. When you get to the statement that says that the rest of the questions are for office use only, then click on the "OK" button. The dialog box will disappear, and all of your answers will appear in the correct places on the form. You can always edit your answers by typing "ATE" again, clicking "Enter", then click the form to bring back the dialog box. It will have all of your answers as you typed them. Just change the answers that need to be changed, and click "OK" to close out of the dialog box. Be sure to save or print a copy of the form before sending it off to me. If you have any question about whether or not a permit is required, then please call 207-626-3889 or email eric.j.ellis@maine.gov. [Last updated 1-1-08]
