The Maine Department of Education regularly collects data, applications and other types of reporting forms from the state's school districts related to the administration of all types of programs.
This page lists the various reporting forms associated with programs in the following areas. Click on the program type to view the list of related forms. Each form includes instructions for submission to the Department of Education.
Department staff are actively engaged in efforts to minimize reporting burdens on school districts and streamlining the reporting process.
