Program Approval
All public and private programs that intend to provide special education and related services for children under school age five must obtain approval through the CDS State IEU. The program approval process must be completed prior to a Regional Site entering into an agreement for services.

- Early Childhood Special Education Program Update Form (DOC, 170KB). This form must be completed and submitted to the CDS State IEU when any change occurs within an approved program. Additional Preschool Classroom Profile (DOC, 84KB) if needed.
- Early Childhood Setting Profile (DOC, 197KB). This form must be completed by those programs we are interested in obtaining an Early Childhood Tuition Agreement (formally known as Community Placement Contracts). Additional Early Childhood Classroom Staffing Profile (DOC, 78KB) if needed.
