Harvester Form:

Dealer Forms: Dealers can choose from 7 different reporting options; 3 are electronic reporting options and 4 are paper forms (see links below). For more information on reporting electronically, contact Heidi Bray (see below for contact information). One electronic reporting option comes with FREE accounting capabilities for your business.

*Form 2006.4A may be used if you ONLY buy lobster and/or crabs from harvesters. If you buy other species from harvesters you will need to choose a different form.
