Military Programs

Individuals will complete the program available in their branch of the service, and the program must include a clinical substance abuse assessment component. Any recommended evaluation or treatment services must be satisfactorily completed.

• Fee = $150.00 payable to DEEP (includes a non-refundable registration fee of $75.00)
• Payment by Money Order or Credit Card only, with a $10.00 processing fee for credit card payment
• Please include full name and date of birth on the money order
