Consent for Release of Confidential Information: Word | PDF

This form gives the DEEP office permission to release paperwork or other information regarding a client’s status with DEEP to a third party, such as a counselor, agency, family member, or attorney. Because of a strict federal law, DEEP cannot share any information with a third party without this specified written and signed consent.

Out of State Release Form: Word | PDF

This is the same consent form as above, but is written to fit the needs of those who must have information released to another state.
