Information for Patients: Maine's Prescription Monitoring Program

Patients may obtain access to their own information in the PMP database. The most common way for patients to find out what prescriptions have been attributed to them in the database is by their health care providers.

A patient, or a patient's authorized representative, may also obtain a report listing all prescription monitoring information that pertains to the patient directly from OSA. The procedure involves presenting a writen request in person at the Office of Substance Abuse. Proof of identification and proof of legal authorization (if a representative) are required.
