Primary Care Case Management (PCCM) Enrollment/Disenrollment Report now available for PCCM providers, CR 28628

The PCCM Enrollment/Disenrollment Report is a bi-monthly report that lists PCCM members who have been added or removed from your Panel Report in previous weeks. For example, the report run on 2/3/14 captures PCCM members who have been added or removed from the panel for dates 1/16/14 – 1/31/14. Another report will be generated after mid-February to show which PCCM members were added or removed for dates 2/1/14 – 2/15/14.

This report supplements the PCCM Panel Report, which is a snapshot of all PCCM members assigned to the PCP or practice as of the 15th of each month.

Both the Enrollment/Disenrollment Report and the Panel Report can be found on the MyHealth PAS Online Portal by signing in to the secure Trading Partner page and going to: File Exchange>Reports>PCP Roster Report. The file name for the Enrollment/Disenrollment Report begins with PCCM0021. The file name for the Panel Report begins with PCCM0022. Both file names also contain the NPI and report run date. Providers that do not have a Trading Partner account will have these reports mailed to them.
