Prior Authorization (PA) Forms for Chronic Pain Management are Now Available
The following forms are now available in the Authorizations and Referrals folder of the Provider Forms page of the Health PAS Online Portal:
• PA for Physical Therapy (PT) Services for Chronic Pain Form • PA Fax Cover Sheet for Therapy Services Review
