Denied Claims with Procedure Code H2021 and a Modifier

There was a known issue where claims with code H2021 and a modifier were denied in error for the following reasons:
- MIHMS Edit 271- Benefit Restriction Group Validation Failed
- CARC B5 - Coverage/program guidelines were not met or were exceeded
This issue has been resolved and all affected claims will be re-processed. Providers do not need to re-submit these claims.
