Claim denials due to “Invalid Discharge Hour”

Providers who submit institutional claims using the UB-04 form are experiencing denials based on the claim discharge hour. This is happening because when you submit an 837I, and discharge time begins with a zero, the leading zero is dropped and the next two digits are stored as the hour. Example: 0900 should be stored as '09' but it's being stored as '90'.

The denials will appear: - In MIHMS: as edit 309 “invalid discharge hour.” - On Remittance Advice (RA) as CARC 125 “Submission/billing error(s).” - On Remittance Advice (RA) as RARC 318 “Missing/incomplete/invalid discharge or end of care date.”

This issue is currently being addressed. Affected claims will be reprocessed. You do not need to resubmit these claims.
