Revised Well Child Visit Forms Process Change

Revised Maine-specific Well Child Visit forms (sometimes called “Bright Futures Forms”) are now available on the MaineCare Children’s Services webpage. These forms replace the Well Child Visit. Now that the revised forms are available, the process of ordering them through Goold Health Systems (GHS) is discontinued. Instead, practices can download, save, and print the forms, as needed.
