Notice to DME Providers
A Durable Medical Equipment (DME) “Repair/Replacement Justification Form” is now available for use to expedite the processing of prior authorization (PA) requests for repair/replacement parts.
The form will be posted for downloading on the HealthPAS Provider Portal.
Please submit this form with your PA request for repair parts.
