Check Provider File Details on the Portal

Providers may want to go to the portal to check the details of their enrollment to verify service locations and rendering providers. To do this, providers would go to Health PAS-Online and choose the ‘Check Status of Enrollment App’ tab found under Provider Enrollment Links (the page that opens is named ‘Resume Enrollment’, yet you will actually only be viewing your enrollment; no changes either intentional or accidental that are done here will NOT be saved.
When entering the email address, case number and tax ID on the first page, please use the most recent case number (which would be different from the original enrollment case number if you have completed any maintenance cases since enrollment). Please call the EDI Helpdesk at 1-866-690-5585, Option 3 if you don’t know your most recent case number.
