  There have been many questions from home health providers regarding the requirements in Section 40 for therapies and the documentation that meets or complies with the requirements. The Rehab Potential Form (above) was created by the Department to assist providers in meeting these requirements. The form is not required by policy. The Department has created the form as a tool to assist providers in meeting the requirements and outlining for physicians what is required of them when ordering a therapy service. The Department when receiving HCFA 485’s and admit/start of care forms will use the form as their review tool to make sure that the information is available on submission of the packet for any therapies provided under Section 40.
