Home care: Where to find ItMedical Eligibility DeterminationThe MED is a form used when people ask for long-term care services. The letters M-E-D stand for Medical Eligibility Determination. The form is filled out by a nurse and looks at a person’s abilities and limitations. After doing the MED the nurse will tell you whether or not you are medically qualified for certain services and let you know what other kinds of care might be available to you. The nurse is also able to tell you if you are likely to be financially eligible for MaineCare. The nurse will give you an application that has to be sent to the Office of Intergrated Access and Support. Financial eligibility for MaineCare services is determined separately by the Office of Intergrated Access and Support. Having a MED assessment can also be useful in planning for services that may be needed in the future. It is free and confidential.
