Choice Letter
If you are assessed (see Long-term Care Assessment) for medical
eligibility for certain long-term care programs by a nurse from the assessing agency, you will
be asked to sign a “Choice
Letter.” If you choose to receive care in your home, for example, rather than in a nursing
home, you would select that choice and sign the “Choice Letter.” Your signature
means that services can start as soon as possible. An agency cannot start providing you with
help until you sign the Choice Letter.
