Chronic Disease Self-Management Program

Living Well is a program for anyone living with a long-term health problem.

  • Workshop given two and a half hours, once a week, for six weeks
  • People with different health problems attend together
  • By taking part in this program you will learn better ways of coping and managing your health:
    • Cope with feelings of sadness
    • Deal with pain and extreme tiredness
    • Eat for better health
    • Talk with family, friends and health professionals
    • Manage medications
    • Relax and enjoy life
