Description:  Amicus Case Management assists individuals with developmental disabilities to access needed services and activities within the community.  The program focusses on connecting people with professional services as well as building upon/developing  natural supports to address identified needs and desires.  Our case managers assist with obtaining benefits for which individuals are eligible and provide the coordination and monitoring of these services.  They work with the individual, their family, and other service providers to establish goals that are person centered. 

Our professional staff members provide guidance, information, support, and links to supportive services including housing, program services, transportation, crisis intervention, employment, and respite.  The case management program for adults with developmental disabilities provides assistance in locating resources, becoming more independent and overcoming challenges of living and being part of the community.

Examples of services include:

  • Assisting in finding medical, social, educational, and other services needed and/or desired.
  • Providing information related to employment opportunities, housing, and skillbuilding opportunities in the community
  • Involving the individual in making important decisions about his/her life based on personal strengths and wishes.
  • Helping the individual grow and feel better about self as he/she makes choices and achieves personal goals.

The ultimate goal of case management is to help the individual gain the highest level of independence and personal growth possible.
