In Home Care

The Department of Health and Human Services offers several programs that provide supportive services to elderly and disabled adults in order to avoid or delay nursing home placement. In-home services are funded by either through MaineCare or through state funds.These services are designed to assist a consumer remain independent in the community, if they so choose. Services include: State Funded In Home and Community Home Based Care; and Medicaid Waiver for Elderly and Adults with Disabilities; MaineCare Home Health Services, and MaineCare Private Duty Nursing Services. Services may be provided through traditional agency or through a consumer directed delivery model, if allowable under the program. 

The State of Maine requires an assessment to determine an individual’s functional and financial eligibility for most In-Home Services. Assessments help individuals and families understand what services are available to them and to plan for service needs. The Medical Eligibility Determination Form (MED)  is the tool approved by DHHS to determine program eligibility for this service.  Assessments are completed  by Goold Health Services.
