Release of Information

If you decide to get help from an agency such as a home health agency, you will be asked to sign a “release of information” form. Signing this form means that you allow the agency to share your medical and other relevant records only with those people, departments, or agencies listed on the form. It also means that you give permission for the agency to get information from the people, department and agencies listed on the form.
